I understand that a tattoo is a form of body modification, made by inserting indelible ink into the dermis layer of the skin to change the pigment. I understand this is a permanent change to my appearance. I have been fully informed of the risks associated with receiving a tattoo. *

I understand that these risks, known and unknown, can lead to injury including (but not limited to) scarring, infection, difficulties in detecting melanoma, and allergic reactions to ink pigment, latex gloves, and/or topical applications such as soap, bactine, and witch hazel. Having been informed of potential risks, I still wish to proceed with the tattoo application. I freely accept any and all risk that may arise from receiving a tattoo.*

I waive and release to the fullest extent permitted by law, the Artist and The Pretty Sister/Wrapped In Plastic INK (hereafter referred to as ‘The Studio’) from all liability whatsoever , including personal injury on premises. For any personal injury or otherwise, including and direct and/or consequential damages which result from the application of my tattoo, whether or not caused by the negligence or fault of either the tattoo Artist or The Studio.*

Neither The Artist nor The Studio is responsible for the meaning or spelling of the symbol or text I have provided them with. Variation in color and design may exist between the art that I have selected and the actual tattoo after it is applied. *

I understand that over time the colors and clarity may fade due to unprotected exposure to the sun and the naturally occurring dispersion of pigment under the skin. A tattoo is permanent and can only be removed by laser or surgical means. This can be disfiguring and/or costly and will not result in the restoration of my skin to it's exact appearance before being tattooed.*

The Artist and The Studio have given me instructions on how to care for my tattoo while it is healing. I understand these instructions and will follow them. I understand that a new tattoo may become infected, and failure to follow these aftercare instructions may affect the healing process and thereby the appearance of the healed tattoo. If any touch up work needs to be done due to my own negligence, I agree that the work will be done at my own expense.*

Both the Artist and The Studio have given me the opportunity to ask questions about the application of my tattoo and all of my questions have been answered to my satisfaction.*

If needed, I have taken the prescribed preventative regimen of antibiotic that is required by my doctor in advance of any tattooing.*

I am not under the influence of alcohol or any other drug. I am voluntarily submitting to be tattooed by the artist without duress or coercion.*

I agree to reimburse the Artist and The Studio for any attorneys fees and costs incurred in any legal action I bring against them in which either the Artist or The Studio is the prevailing party. I agree that the courts of California in Orange County shall have personal jurisdiction and venue over me and shall have exclusive jurisdiction for the purpose of litigating any dispute arising out of, or relating to, this agreement.*

I acknowledge that I have been given adequate opportunity to read and understand this document. I understand that I am signing a legal contract waiving certain rights to recover against the Artist and The Studio.*

I do not have any mental impairment that may affect my judgement and I am capable of entering this agreement.*

I hereby state that I am of legal age, I have provided proof of age, and I am competent to sign this agreement.*

I grant the Artist permission to take photographs of the tattoo for promotional purposes, including but not limited to social media, the Artist's portfolio, and other marketing materials. I release all rights of any photographs or videos taken of me or my tattoo and I give consent in advance to their reproduction and public visibility.
