WAIVER, RELEASE, AND

CONSENT TO TATTOO

Please read this form in its entirety and be certain you understand the implications of signing this form. Once you have read, understood, and consent to be tattooed, please sign the form.

1. DESCRIPTION OF SERVICES

The Artist agrees to perform tattoo services for the Client at the following location: The Pretty Sister Studio - Costa Mesa, CA

2. ASSUMPTION OF RISK

The Client acknowledges that getting a tattoo involves certain risks, including but not limited to infection, scarring, difficulties in detecting melanoma and allergic reactions and dissatisfaction with the artwork. The Client voluntarily assumes all risks associated with the tattoo procedure and freely accepts and expressly assumes any and all risks that may arise from tattooing.

3. RELEASE OF LIABILITY

The Client, on behalf of themselves, their heirs, assigns, and personal representatives, hereby releases, waives, and discharges the Artist, their employees, and agents from any and all claims or liability for personal injury, property damage, or wrongful death arising out of or in any way connected with the tattoo procedure.

4. CONSENT TO TATTOO

The Client certifies that they are at least 18 years of age and legally capable of entering into this Agreement. The Client has provided accurate information regarding their medical history and any allergies or skin conditions. The Client consents to the application of the tattoo as discussed and understands that the final design may vary slightly from the initial design presented. The Artist has given me the opportunity to ask any and all questions about the tattoo procedure and all questions have been answered to Client's satisfaction.

5. AFTERCARE INSTRUCTIONS

The Client agrees to follow the aftercare instructions provided by the Artist to minimize the risk of infection and ensure proper healing of the tattoo. The Client acknowledges that a new tattoo may become infected, and failure to follow these aftercare instructions may affect the appearance of the tattoo and the affect healing process.

6. PHOTOGRAPHY AND SOCIAL MEDIA

The Client grants the Artist permission to take photographs of the tattoo for promotional purposes, including but not limited to social media, the Artist's portfolio, and other marketing materials. The Client may request that their identity remain confidential, and the Artist will respect such requests.

7. SOUND MIND

I am not under the influence of alcohol or drugs and I am voluntarily submitting to be tattooed by The Artist without duress or concern.

8. ENTIRE AGREEMENT

This Agreement constitutes the entire understanding between the parties and supersedes all prior agreements, understandings, and representations. A tattoo is a permanent change to my appearance and can only be removed by laser or surgical means, which may be disfiguring and costly and will not result in the restoration of my skin to it's exact appearance before being tattooed.
